  • Pregnant or breastfeeding → GP / LMC first, regardless of BMI
  • BMI ≥ 30 → eligible (invite to book initial assessment)
  • BMI 27–29.9 with at least one weight-related condition (T2D/prediabetes, hypertension, high cholesterol, sleep apnoea, PCOS) → eligible
  • BMI 25–29.9 without qualifying conditions → borderline (talk to team)
  • BMI < 25 → refer to GP (soft landing, no hard "decline")
  • Age and sex are collected but not used in branching yet — happy to add rules (e.g. under-18 exclusion is enforced by the 18–29 minimum bracket; ethnicity-specific BMI thresholds can be added if clinically preferred)
  • Medication questions deliberately excluded from the online quiz (privacy minimisation) — covered at consultation
